I accept full responsibility for any harm or damage caused by my dog whilst in the care of Jax & friends provided that reasonable measures are taken to protect my dog, and others whilst they carry out their service.

I agree that any information regarding my dog I have disclosed is true to the best of my knowledge and that my dog has not shown signs of aggression and is in good health to be taken on a walk.

I accept that any dogs may be denied admission into Jax & Friends at any given point if the dog is deemed dangerous or in need of a veterinary visit prior to admission.

I accept that I must, at the earliest point possible, notify Jax a Friends if there are any changes to the dog which are relevant to the mood or health of the dog and can affect their behaviour and wellbeing.

Jax a Friends will only let my dog off lead after a 'off-lead consent form' is signed and they will follow their procedure to protect my dog and third parties.

I am responsible for any and all treatment of any injuries sustained by my dog whilst in the care of Jax & Friends. I authorise Jax & Friends to seek medical attention for my dogs if required, preferably by my named vet but I understand this may not always be possible and allow them to use any approved veterinary service.

I accept that although my dog is vaccinated, diseases such as Kennel Cough are still possible and I will not hold Jax & Friends responsible if my dog contracts a disease whilst in their care.

I will take all of the necessary measures to make sure my dog is free from contagious or infectious diseases and will notify Jax & Friends if my dog contracts or is exposed to such diseases. I also understand this may lead to my dogs admission being revoked in this time, until resolved.

I agree that any lost, stolen or damaged leads, collars, tags or clothing items for my dog are not the responsibility of Jax & Friends and as such they will not have to replace these.

I agree that I will not feed my dog in the hour before Jax & Friends will be attending to my dog as I understand this may result in illness or choking for my dog which is potentially life threatening for my dog.

I will provide keys and /or adequate access for Jax & Friends to access my dog on the arranged date and I accept that failure to do so may result in a cancellation and that I will forfeit the full amount of fees for that days service.

No changes may be made to these Terms and Conditions without the written instruction from Jax & Friends.
